WebCould be a crack in the cylinder head from the fuel return passage to a coolant passage. As others have suggested, it’s a cylinder head problem, either a crack, or pitting around the injector cups. It’s pushing coolant into the fuel return, and it’s getting returned to the tank and sucked back up, and caught by the separator filter.

It does not appear to be leaking anywahere, but when the truck is running, you can smell hot coolant around the passenger side.
